Start the application process online:

  1. Download and complete the application form
  2. Save the form on your computer, or print it to save a hard copy
  3. Send the application to us in one of the following ways:
  • Email the attachment to loans@rocmon.org
  • FAX to 585-546-3745
  • Mail us your printed copy:
